1. This discovery call will take place via a video teleconferencing platform. (If you would prefer to conduct the discovery call by phone instead please let us know.) During your call we will ask you some questions to determine your concerns and recommend next steps. Please let us know during this call if you are interested in getting reimbursed by your insurance company. Insurance reimbursement is NOT guaranteed but we are happy to help you determine in advance if insurance is likely to help cover any expenses. You may schedule your evaluation during the course of this initial call. However, if you have insurance, we will likely recommend you contact your insurance company prior to scheduling your first appointment.
2. If interested in having insurance reimburse some of your therapy expenses please call up your insurance company BEFORE your initial evaluation so that there are no unexpected surprises in cost following the start of therapy. Functional Foundations Speech Therapy, PLLC is not responsible for making sure your insurance will provide reimbursement. The patient is responsible for making sure the services are reimbursable with their insurance company. However, we are happy to assist you with this process to make sure that you can afford the treatment you are seeking prior to beginning therapy. 3. When ready to schedule your initial evaluation please get in contact at info@functionalslp.com. Following this contact you will be sent any intake paperwork which is needed prior to your evaluation.
4. Please fill out all requested intake paperwork and submit it to the patient portal at least 48 hours prior to your scheduled appointment. (Included with this will also be instructions for items you may need to have available at the time of evaluation.) The sooner we receive your completed paperwork the better. This allows the therapist time to review the information and plan the evaluation session accordingly.
